RAN Therapy in siddha for AIDS

RAN' - SIDDHA THERAPY

RAN this acronym stands for 1Rasagandhi mezhugu-R, 2Amukkara chooranam - A and 3Nellikkai Ilagam - N, now the most popular Siddha combination for HIV in Tamil Nadu having Immunogenic and adaptogenic properties. RAN therapy is given to HIV patients in the Asia's biggest AIDS hospital at TB Sanatorium, Tambaram, Chennai, South India .

Deivanayagam and co-workers in 2001 could establish laboratory evidence supporting the clinical improvement in more than 60 % of the patients who received either RAN alone or in combination with opportunistic infection (OI) controlling drugs. But RAN could not save those with brain infection, or septicaemia.

Since April 2002, the family health clinic of Health India Foundation (HIF), a registered non-governmental charity, has been running an out-patient service for HIV/AIDS at Red Cross Hospital, Chennai. The author, during his tenure as medical officer at HIF for 13 months have himself closely observed and monitored the safety and effect of RAN therapy in patients with mild to moderate HIV disease (Wilson, 2010). Fritts et al., 2008 in his review article from USA have included the pilot trial conducted with RAN therapy.

Note

2 Poly herbal medicated powder prepared using Withania somnifera Dunal (winter cherry) as the chief ingredient.

Divine part of Siddha system of medicine

Siddha system of medicine aims at reconciliation of the earthly and divine. This attitude is characteristic of Siddha medicine throughout its evolution from time remote and to all its level. The first medical treatise was by Siddhanar or Sivanar. It was followed innumerable works  of Nandhi, Agasthiyar, Pulathiar, theraiyar, Pogar, Senkanar etc. The means of curing disease according to Thirumoolar is by simple herbs, salts, Theeneer, upavasam, padanam, ulokam, sathu, rasa kulikai and yoga. The three humors vatham, pitham, kapham  serves as three connecting link between Microcosm the man and Macrocosm the Universe. SIDDHA system treats not the disease alone but man as whole.

siddha Nei kuri neerkuri workshop

💎Ezhaam Thervu 2018🔎🔍
Conference on 17-18 November 2018
Will be Enjoying the depth of Siddha Pathology💡

Neikuri - Therayar Methodology of Uroscopy

Procedure

15 µl of Black Dil oil is put over the surface of 200 ml of voided and cooled test urine kept in a four inches diameter round disposable pet bowel from 5 centimeters height. The reaction of oil drop on the urine surface is carefully observed accordingly on dispersion, spread, split, immersion, floating, disappearance by from the time of putting the drop up to one minute or the maximum spread reaches.

The test may br done in a chamber maintained with 29º C to 31º C temperature and 3-5 Watts LED luminance to maintain the classical test standard.

Observation

The reactions occurred over the surface of the urine is observed carefully. The changes in the shape of oil after spread, direction of spread or split, coverage area on the urine surface, immersion are observed.

Regarding the various patterns of spread of Dil oil on the urine surface, as per Therayar Neikuri Methodology as a diagnostic aid it may be suggested that the surface shows a snake (Vali), a circular shape ring (Azhal), a pearl without any movement (Iyam) and combined patterns ring in the snake (Vali Azhal), snake in the ring (Azhal Vali), pearl in the snake (Vali Iyam) and pearl in the ring(Azhal Iyam) and the mixture of all the patterns (Mukkutram).

The various patterns (approximately 136 patterns) such as Conch-shell, Throne, Umbrella, Lute, Jasmine bud, Lotus, Bubbles, Scattered mustard, Arrow, Trident, Sword, Pot, Iron pestle, Betel leaf, Bullock, Lion, Elephant, Tiger, Goat, Pig, Man, Snake, Tortoise, Bird, Jackal, Monkey, Donkey, Cat, Scorpion are correlated with clinical features and other investigations by a Siddha Physician.

Interpretation

The directional spread of oil may be explained on the basis that there may be presence of the paramagnetic molecules which may align as per the magnetic field of the earth, giving a directional spread to the oil. The interfacial tension between the surface active molecules and the oil may decide providing possibilities of different shapes, speed, and extent of spread. The constituents of test urine depend upon the existing diseases, dietary metabolites, major and minor constituents excreted through urine and microorganisms.

Presuming the interference of the constituents of urine with the reacting Dil oil drop in a standard procedure may be evidence for existing diseases according to Therayar Neikuri Methodology.

Viscosity of the urine, Interfacial tension, Quality of the Dil oil, Air entry in the oil drop or urine volume, Shape of the drop, Gravitational force, Shake of the chamber, Quantity of the urine if very lesser are the factors cause variations for reaching test results.

Impression

According to the observation and the available image it may be suggested that the surface shows a snake (Vali), a circular shape ring (Azhal), a pearl without any movement (Iyam) and combined patterns ring in the snake (Vali Azhal), snake in the ring (Azhal Vali), pearl in the snake (Vali Iyam) and pearl in the ring (Azhal Iyam) and the mixture of all the patterns (Mukkutram).

Siddha medicine @ WHO

It is a memorable day  for representing Siddha to the Apex body for health in the world (WHO)

In the recent past, many milestones have been achieved only because of some organic way of work execution and system are in place in terms of huge development in the technical and technological part.

Ministry of AYUSH has arranged a consultative meeting with WHO professionals from H.Q (Geneva) and SEARO Regional office, New Delhi at AYUSH Bhawan, Ministry of AYUSH, Govt. of India, New Delhi. Each ASU system has to present their current work on morbidity codes. Dr. S. Natarajan and Dr. M. Kannan represented the Siddha system of Medicine in the WHO-ICD team meeting. With of vision of including AYUSH under International Classification of Disease - ICD 11 - Traditional medicine (TM) chapter, Ministry of AYUSH has initiated a project of creating a nationwide uniform morbidity code for AYUSH. Ayurveda, Siddha, Unani systems of medicines have been asked to create their national morbidity codes.

The NSMC was developed by the CCRS working group and evaluation & guidance provided by the Expert committee formed by the Ministry of AYUSH. Govt. of India. This has two parts – one is Standardized Siddha Terminologies (SST) which was developed and published by the National Institute of Siddha, in May 2014. The other one is the NSMC which was prepared by the CCRS, which all are available in the National AYUSH Morbidity codes and Standardized Electronic Portal (NAMASTE) – www. Namstp.ayush.gov.in.

Siddha  team  presented the topic entitled  “ Current status of Documenting and Coding of SIDDHA diagnostic and intervention related information” with the following points—Indian scenario, Global Scenario,  Education and practice, Documentation, Patient safety,  Stepwise process of development of Standardized Siddha Terminologies through WHO funded project and published by NIS,  Genesis of Morbidity codes available in CCRS (Numeric code, Alpha Numeric code and finally NSMC), Stepwise process of development of National Siddha Morbidity Codes, Ontology Framework of NSMC by the working group of CCRS and Expert committee, Hospital Information Management System, Statistics, Reimbursement/insurance,  End-user and Use case scenarios, Strength, Challenges and limitations of the NSMC, Integration of NSMC, Hospital setting – Primary, Secondary and Tertiary level of Hospital with implementation of NSMC.  We highlighted that we have a real-time data capturing system with THE Research Application Nexus (THERAN). They insisted on Electronic Data Capturing System to document as a package and it is essential for ICD. (Now THERAN reaches to the national level as A-HMIS for the all the AYUSH systems).

WHO personnel has appreciated the contribution and they have given many suggestions to fill the gaps for further development of NSMC to include in the ICD11-TM.  Finally, they said about the level of standardization is overall good and in place. They are really looking forward to onboard activities of National morbidity codes for ASU.  In this track Ministry of AYUSH and CCRAS has taken a lot of pain to launch the NAMASTE portal. On November 2017, Honorable PM has launched the NAMASTE. AYUSH morbidity is being collected through this portal. On the pilot basis, CCRS Institute is using this NSMC and uploading to NAMASTE portal. Dr. T. Saketh Ram, RO (Ayu) has contributed to building the entire site and maintenance.

Purification process of Raw drugs in siddha system of medicine

Purification is a process mentioned by Siddhars which holds  lot of Scientific information. Very commonly used is Kadukkaiku Aga Nanchu which has been Scientifically validated. This Research provides an overview of the purification methods of Terminalia chebula (TC) as mentioned in Siddha medical practice (2), a traditional medical system in India. According to this practice the purification of the herb is to remove the seed from the ripen fruit before processing into medicine. Changes in the chemical profile of Terminalia chebula before (with seed - TCS) and after purification (without seed - TCWS) is achieved through physiochemical, phytochemical and HPTLC analysis. The results revealed TCWS has 60% water soluble extract and 40% alcohol soluble extract to that of 25% and 15% respectively in TCS. The total ash content was very minimal in TCWS. There is increase in the concentration of tannins, anthraquinones, alkaloids, saponins and decrease in the protein content in TCWS. No change in the concentration of total phenols, flavones, glycosides, reducing sugar and quinines. The HPTLC analysis shows nine peaks for TCWS whereas seven for TCS. This study reassures that Terminalia chebula is pharmaceutically potent only after removal of seeds which substantiates intuition of Siddha’s scientifically.

Thanks for all the Scientists who has contributed to this paper.

Ethics in Siddha Medical Literature

Ethics in Siddha medical literature:

In Siddha medical literature, ethics of the physician –patient relationship are available scattered, rather than all together in a single place.

Siddhar Agasthiyar in the classical Siddha medical text ‘Agasthiyar paripooranam 400” mentions little code of medical ethics, as follows,

• ‘Virtue lies in treating a poor, and not in treating a king’. (Verse 370).
[The verse has earmarks of the World Medical Association Declaration of Geneva (amended by the 68th WMA General Assembly, Chicago, October 2017), ‘I will not permit consideration of age, disease or disability, creed, ethnic origin, gender, nationality, political affection, race, sexual orientation, social standing or any other factor to intervene my duty and my patient’]

• ‘Treat others the same way you want them to treat you’ (Verse 371)
[A most inclusive line captures all the codes of medical ethics]

In these two verses, Agasthiyar repeatedly stresses, ‘These virtues will make the physician mastery in her / his profession’.

Manjatti kudineer=: siddha drug for non healing wounds

As per Siddha literary text, the non healing wounds are termed as Kaaliga viranam (காலிக விரணம்). Many effective treatments for the facilitation of wound healing is available in Siddha medicine of which Manjitti kudineer (மஞ்சிட்டி குடிநீர்) has been recently studied at National Institute of Siddha.

It has been observed that on administration of Manjitti kudineer along with usual wound hygiene causes the wound margins to shrink and close in a short span of 15-90 days regardless of the wound chronicity in the majority.

This formulation in particular serves as a powerful agent to contract the wound margins and eventually close to heal it. Even wounds of several years duration (i.e., 30 years) had been healed in 3 months time.

Various types of wounds caused due to venous (varicose) and arterial insufficiency, diabetes, trauma etc., respond well to the above Siddha treatment.

Wounds as big as 10cm dimension also get healed with this regimen of treatment. Study showed that the Manjitti Kudineer was effective in enhancing the Wound contraction and epithelialization.

Manjatti kudineer in siddha

Longevity -siddha medicine Triphala powder

Siddha herbs not only give a longevity but also provide disease free life if someone starts early adult age.
E.g. Nellikani (Amla) containing THIRIBALA POWDER with honey is the best supplement to avoid oxidative stress in our body. It should be taken daily with consulting siddha doctor.
Amla contains disease preventing/fighting the most bioavailable sources of antioxidants. Researchers discovered amla fruit significantly reduced oxidative stress while vitamin C alone had no such advanced effect.
Oxidative Stress?
Imbalance between the systemic manifestation of reactive oxygen species and a biological system's ability to readily detoxify the reactive intermediates or to repair the resulting damage.
Body generates reactive oxygen species known as ROS during normal metabolism.
Chemicals, pollutants, abnormal physical and mental activities further accelerate this ROS levels either directly or indirectly.

We could live healthy life with out non-communicable disease, if we effectively block the oxidative stress happening in our body every seconds.
Human body some how manages the Oxidative stress by the sources of internal enzymatic and external antioxidant sources including micro nutrients mainly from fruits. Oxidative stress levels increases during the aging process. Aged people need more external antioxidants to prevent disease. Cactus fruit is one of the cheap and best option.
The opinion expressed here is just share the knowledge not a medical advice. Here I just express the importance of ancient medicine and need to be developed properly by the government research institutes in order to benefit public. Siddha sciences should be added to one of the subject branch in the school education.
